Parkinson’s Care Navigation: Specialist Coordination, Medication Timing, Mobility, and Caregiver Support
Introduction
Parkinson’s disease (PD) care is complex: multiple clinicians, dynamic medication schedules, functional changes, and social needs. Aviator Health pairs each patient and family with a dedicated Healthcare Navigator to coordinate specialists, therapies, durable medical equipment (DME), transportation, and benefits—often at $0 out-of-pocket for Medicare and Medicare Advantage members. Navigators provide structured visits plus unlimited messaging and can join appointments virtually to ensure questions are answered and tasks are completed. Aviator Health, Mobility support.
What your Navigator does for Parkinson’s
-
Coordinate neurologist and primary care visits; prepare agendas; track referrals and prior authorizations. Aviator Health
-
Orchestrate therapy plans (physical, occupational, speech) and follow-ups; align appointments with medication “on” times per clinician guidance. Aviator Health
-
Educate on medication timing per the clinician’s orders, build daily schedules and reminders, and escalate concerns to the prescriber (no medication changes by Navigators). Terms of Use
-
Secure mobility/DME (walkers, rollators, wheelchairs, scooters) and accessories; manage documentation, supplier coordination, and coverage steps. Mobility, Walker vs. Rollator
-
Address social determinants (transportation, food access) and connect to local resources and benefits (e.g., SNAP, home-delivered meals). Food support
-
Support caregivers with education, respite options, and care coordination to reduce burden and prevent crises. About Aviator
Neurologist and therapy coordination
Aviator schedules and sequences care across neurology, primary care, rehabilitation, and community resources, then tracks execution between visits. Navigators can join telehealth or phone appointments to take notes, confirm instructions, and capture next steps and orders. We coordinate:
-
Physical therapy for gait, balance, and fall prevention; occupational therapy for ADLs and home adaptation; speech therapy for voice and swallowing—as ordered by your clinicians. Aviator Health
-
Post-visit logistics: referrals, prior authorizations, transportation, and plan-of-care documentation. Mobility
Medication timing education (non-clinical)
Medication timing is central in PD. Aviator helps you follow the regimen your clinician prescribes by creating clear, personalized schedules and automated reminders and by coordinating around “best function” windows for therapy and medical visits. We do not diagnose, prescribe, or change medications; clinical care is delivered by Aviator Medical Group PA where applicable. Terms of Use, HIPAA Notice
Mobility and home safety resources
Navigators streamline DME procurement and home safety planning:
-
Determine appropriate devices (e.g., standard walker vs. rollator) with your therapy team; ensure proper fit and training; align documentation for coverage. Walker vs. Rollator, Mobility
-
Coordinate equipment trials when possible, accessories (seating cushions, trays, brakes), and supplier delivery and follow-up. Mobility
Caregiver guidance and support
PD affects the whole household. Navigators provide practical education, organize resources (transport, meals, home care), and support benefits navigation. For MA supplemental benefits (transportation, meals, flex cards, etc.), we help verify what is truly covered and how to use it—avoiding common pitfalls and misleading marketing. Insurance-covered supports (Medicare/MA)
-
Principal Illness Navigation (PIN) is a 2024 Medicare benefit for patients with serious, high-risk conditions expected to last ≥3 months. Services include person-centered care planning, health system navigation, resource referrals, and self-advocacy support; PIN requires an initiating visit by a billing practitioner and is billed monthly via HCPCS G-codes. RHCs/FQHCs may bill starting in 2025. Coverage specifics and any cost sharing depend on plan and setting. AAFP PIN codes, RHIhub PIN overview

Aviator’s navigation is frequently covered by Medicare and Medicare Advantage, resulting in $0 out-of-pocket for most patients. Benefits vary by plan and provider contracts. Aviator Health
